The admission measure counts patient stays in which the hospice completed all applicable care processes at admission: treatment preferences, beliefs and values, pain screening and assessment, shortness-of-breath screening and treatment, and a bowel regimen for patients on opioids. The last-days measure comes from Medicare fee-for-service claims for patients who died in hospice care and counts in-person visits by a registered nurse or medical social worker. It leaves out patients enrolled fewer than 3 days and patients who got continuous home care, respite or inpatient care in their last 3 days.
