About this site and its data
Nursing Home Checker puts the public record of every Medicare- or Medicaid-certified nursing home in the United States on one page per home. It is an independent site and is not affiliated with or endorsed by CMS, Medicare or any government agency.
Where the data comes from
All figures come from the CMS Provider Data Catalog, the same source behind Medicare.gov Care Compare. The site uses these nursing home datasets: Provider Information, Health Deficiencies, Survey Summary, Penalties, and State and US Averages. The current figures were processed by CMS on Aug 1, 2026. The site rebuilds when CMS publishes a new file.
What this site calculates
- Certified beds in use: average residents per day divided by certified beds.
- Size rank: a home's position by certified beds among homes in its state.
- Nearest homes: straight-line distance between the coordinates CMS publishes.
- County totals and averages: sums and simple averages of the homes listed in that county.
- Questions to ask: chosen by fixed rules from each home's figures, for example when turnover is above the state average.
Everything else is shown as CMS published it. Names and addresses that CMS lists in capital letters are shown in ordinary capitalization.
What this site does not show
- Prices. CMS does not publish what homes charge.
- Today's openings. Bed use is an average, not a live count.
- Names of individual owners. CMS publishes them; this site lists only the ownership type and chain.
- Assisted living. Those facilities are licensed by states and are not in this CMS data.
How this site is funded
The site is free to use, with no accounts. It does not take referral or placement fees from nursing homes, and a home cannot pay to change what is shown about it. If the site carries advertising, the ads are labeled and have no effect on the data.
How the guides are written
The guides are written from official sources: Medicare.gov, CMS, federal regulations and state agencies. Each guide lists its sources and the date it was last checked. Dollar figures that change every year are updated when the new figures are published. State rules are added one state at a time, from that state's own regulations.
Limits
Homes report some of this information themselves, and inspections reflect the days inspectors were on site. A record can lag behind recent changes. The site does not rank or recommend homes, and nothing here is medical or legal advice. Confirm details with the home and check the official record on Medicare.gov before you decide.
