How we calculated this
- Fields: "Penalty Date", "Penalty Type" (Fine or Payment Denial) and "Fine Amount" in CMS's Penalties file for nursing homes.
- Period: penalty dates from September 2, 2025 to September 1, 2026, the 12 months up to and including the processing date of the Provider Information file this site uses. The earliest penalty date in the current file is September 17, 2023.
- Each fine or payment denial is one row in CMS's file; a home can have more than one. Total fines adds the "Fine Amount" of each fine as CMS published it.
- Rows for homes that are not in the Provider Information file are not counted (none in this period).
- These figures describe penalties CMS recorded, not what happened at any one home. To see one home's penalties, open its page on this site.
- Each home is counted in the state listed for it in the Sep 2026 Provider Information file. The table has a row for each place where that file lists homes: 50 states, the District of Columbia, Guam and Puerto Rico.
