How we calculated this
- Field: "Special Focus Status" in the Sep 2026 Provider Information file. Values counted: "SFF" (Special Focus Facility) and "SFF Candidate". Homes with no value are counted only in the Homes column.
- A candidate is a home that qualifies for the program but has not been selected. What the program is and how CMS selects homes is explained in this site's guide to Special Focus Facilities.
- This is a snapshot of one month's file, not a count over time.
- Each home is counted in the state listed for it in the Sep 2026 Provider Information file. The table has a row for each place where that file lists homes: 50 states, the District of Columbia, Guam and Puerto Rico.
